What to Bring?
When you visit our office for the first time, we will need to collect a few items from you. Please refer to the list of forms and documents to bring with you:
- Photo ID, such as a valid driver’s license
- Insurance card (if applicable)
- Recent dental records and X-rays or the contact information to your former dental practice
