Payment and Insurance Information
Our office is a contracted ("in-network") provider with Delta Dental, Met Life and carriers affiliated with Connection Dental. We accept most major PPO insurance carriers and will gladly file your insurance claim for you. Any "co-pays" and/or deductibles will be due at the time of treatment. Pre-estimates of treatment are available upon request.
